Annual Review System
If you are on repeat medication/s, your items will be reviewed every year by our Practice Pharmacist will then contact you to review your medication with you and reauthorise it as appropriate.
You may be offered a health check including blood tests, a specialist nurse review of your long-term conditions, a medication review by a pharmacist or doctor, and a complex care review if you have specific needs.
Housebound Annual Review System
You are housebound if you are unable to leave your home. This means that your annual review tests will take place at home, usually by the community nursing team.
The practice pharmacist will then contact you to review your medication with you and reauthorise it as appropriate.
Your GP may then contact you to review your current health issues and plan for the next year.
Home visits
The doctors can carry out home visits for patients who are housebound and who are more vulnerable.
As soon as possible to arrange this so that we can assess the need for a visit the same day. However, this is at the doctor’s discretion and they will assess your situation over the telephone and advise you appropriately.

Flu Vaccination
Influenza vaccination is particularly recommended for patients with heart, lung or kidney disease, diabetes, for residents of nursing and rest homes, pregnant ladies and people over a certain age depending on the recommended guidence that year. Invitations will be sent out via text message or a letter from the end of September. Home visits can be arranged for housebound patients.
Travel Vaccinations
You will need to complete a Travel Risk Assessment Form at least 6 weeks in advance of your holiday, to make sure that you are well protected. You will then be contacted by the Practice to arrange your appointment/s. If you leave insufficient time we cannot guarantee that the vaccinations will be effective, or that we will be able to fit you in.
Most vaccines are available on the NHS, but for those that are not, we advise you to attend a pharmacy that offers travel vaccinations.
Childhood vaccinations
We follow the national childhood vaccination programme, which starts at 8 weeks of age, in order to protect your child from serious childhood illnesses.

Baby checks and Postnatal Health checks
After birth, your baby has a physical health check by your midwife. We repeat this check between 8 and 10 weeks of age, to check for any health problems, and usually perform the first set of vaccinations at the same appointment.
We like to review the mother's health at around the same time as the baby check. This appointment is to review your physical and emotional recovery after birth and to address any contraceptive needs. But if you do feel like you need to speak to someone or see someone before this please contact the surgery.
You will automatically receive an appointment letter for your Postnatal check and your baby's first immunisations.
Contraception
A full range of contraceptive services are available, including emergency contraception within 72 hours of unprotected intercourse. Coils and Implants are fitted by Dr Maher. Please see a GP to discuss and arrange an appointment.
We currently offer STI self-testing kits. To claim yours, please ask at Reception.
Healthy Start Scheme
The celebrated international footballer and child food poverty campaigner, Marcus Rashford, has called on healthcare professionals to help spread the word about a government scheme designed to give young children in low-income households in England, Wales, and Northern Ireland a nutritious start in life.
The Healthy Start scheme provides vouchers to support pregnant women or those with children aged under 4. If you live in England, Wales or Northern Ireland you may be able to claim free vouchers, or payments every 4 weeks to spend on:
- Fresh, frozen or tinned fruit and vegetables
- Fresh, dried, and tinned pulses
- Cow’s milk
- Infant formula milk
You can also get free Healthy Start vitamins. Healthy Start food vouchers are sent out by post every 4 weeks and Healthy Start vitamins voucher every 8 weeks.

How to Apply
Blood tests
We have a blood test appointments available every week, as well as more in-depth annual review health checks which also involved blood pressure, weight, urine and foot checks, as required. However your blood test must be requested by a clinician - therefore you must speak to a GP before arranging a blood test.
You can also have your blood test at the hospital or at the drive-through centre on Longley Lane. For this option, we need to request your tests in your medical records first, and you will need your NHS number with you.

Emergency Contraception
The Practice offers emergency contraception, which can be taken up to 72 hours after having unprotected sexual intercourse.
If there are no routine appointments available with us, you can obtain emergency contraception from a Pharmacy.

Sexual Health Sheffield

Carers
If you care for a family member or friend with a disability, illness or frailty, or a child with special needs please inform the doctor or nurse when you see them. We like to highlight this in your notes so we can offer you support and flexibility when you are accessing our services. We also suggest you register with the Sheffield Carers’ Centre
Social Prescribing Link Workers
Social prescribing involves helping people to improve their health and wellbeing by connecting them to activities in the community. Link Workers connect those feeling lonely, overwhelmed or in need of help to a range of local support, from community and activity groups to work, debt or housing advice.
They can help with; getting people to focus on their own priorities and the things that affect their wellbeing, supporting people to take more control of their health and introducing people to groups and activities in their community.
Cancer Screening
The NHS provides Cancer Screening services to pick up cancer early to give you the best possible outcome.
We perform smear tests at the medical centre, and take great care to make these as comfortable and acceptable as possible for you.
You will receive an invitation through the NHS screening programme to attend the Royal Hallamshire Hospital for your mammogram, usually every 3 years from the age of 50.
You will receive a bowel cancer screening kit through the post every 2 years from the age of 60. If this is not returned, we will be notified, and we will attempt to contact you to encourage you to do the test.
Mental Health Appointments
We offer mental health assessment, therapy and support through the IAPT service, for first line options.
We are delighted to be able to now offer a variety of additional mental health services through our Emotional Wellbeing team and our Primary Care Mental Health team.

Social Prescribing Link Workers
Link workers give people time, focusing on ‘what matters to me’ and taking a holistic approach to people’s health and wellbeing. They connect people to community groups and statutory services for practical and emotional support.
Link workers also support existing community groups to be accessible and sustainable and help people to start new groups, working collaboratively with all local partners.
You can be referred to our network link workers, Alex Thomson (for anyone over the age of 18) & Matt Astbury (for any young person over 13) by any member of our team.

Changes to Our Ear Irrigation Service
We want to let patients know that the practice will no longer be offering ear irrigation.
Ear irrigation is not part of our NHS contract, and we are not funded to provide this service. Due to increasing financial pressures across general practice, we need to focus our resources on the core services we are required to provide. This means, unfortunately, we are no longer able to offer ear irrigation within the practice.
Alternative options are available locally, including community ear care services, self-funded treatment through some pharmacies, and NHS referrals when clinically appropriate. If you are unsure where to go, our team can advise you on suitable next steps.
